Revenue has fallen every year since 2020. The stock just touched its 52-week high. Those two facts should not coexist, and the gap between them is the whole story.
Biogen spent most of the last decade as a multiple sclerosis company that kept promising it would become something more. The MS franchise was a cash machine, spinning off billions while the company chased an Alzheimer’s cure that kept slipping away. Then Aduhelm arrived in 2021, briefly sent the stock above $468, and collapsed under Medicare’s refusal to pay for it. The revenue line has been falling ever since. What’s changed in 2026 is that the market, for the first time in years, seems to believe the next chapter is real.
First, the shape of it
The filed numbers show a company whose revenue has contracted from $13.4 billion in 2020 to $9.7 billion in 2024, a drop of more than a quarter in four years. The most recent quarter shows 11% growth year-over-year, the first genuine acceleration in that stretch. The stock trades at a price-to-earnings multiple (the price tag the market puts on each dollar of profit) near the 98th percentile of its own decade-long range, our data show. The math says the business is just beginning to stabilize; the mood, meaning what the market is currently paying, prices in a full recovery already. Those two readings cannot both be right, and that disagreement is the story.
How a company loses a third of its revenue and keeps its lights on
The MS franchise that built Biogen is eroding in the way all pharmaceutical franchises eventually do: patents expire, generics arrive, and the revenue that once felt permanent quietly drains away. MS revenue has declined substantially over the past several years, as legacy products face generic competition. A generic version of Vumerity, one of Biogen’s MS pills, received FDA approval in November 2025, adding another drip to that leak.
What kept the company solvent through the decline was operating cash flow, which the 10-K for fiscal year 2025 puts at $2.2 billion for the year. Think of it this way: even as the top line shrank, the business was still generating enough cash each year to pay off a meaningful slice of its total debt load. That cushion bought time to build something new.
The something new is a cluster of drugs the company calls its growth portfolio. In Q2 2026, for the first time, revenue from that newer group, including lecanemab for Alzheimer’s, SKYCLARYS for a rare neurological disease, and products acquired through the May 2026 Apellis deal, exceeded legacy MS revenue. That crossover is the turning point the market is pricing.
What the Q3 filing actually shows
The 10-Q for the quarter ended September 30, 2026 is the most recent filed document, and it contains a genuine surprise alongside a genuine complication.
The surprise: total revenue of $2.8 billion for the quarter, up 11% year-over-year. Lecanemab (sold as Leqembi) generated $202 million in global in-market sales for the quarter, a 67% increase from a year earlier, according to the Q3 2026 earnings call. The Apellis acquisition contributed a further quarter-billion in specialized-immunology revenue. Together, these two lines are doing what management promised: replacing the MS dollars that keep walking out the door.
The complication: Biogen reported a small GAAP net loss for the quarter. GAAP, meaning the accounting standard that counts everything, includes a roughly quarter-billion-dollar charge for acquired research-and-development rights tied to the RayThera acquisition. Strip that charge out and the business looks profitable; include it and the quarter is technically in the red. Both readings are honest. The question is whether the acquisition bought something worth having, and that answer lives in future clinical trial data, not in any filed number today.
The debt picture deserves a moment. The 10-Q for the quarter ended September 30, 2026 shows Biogen carrying roughly $7.7 billion in total debt against $1.3 billion in cash, leaving net debt of about $6.4 billion. At the full-year 2025 operating cash flow rate, that net debt load represents nearly three years of cash generation. That is not a crisis, but it does mean acquisitions have a cost that shows up in the balance sheet for years.
The Leqembi bet, and why the subcutaneous version matters
Lecanemab’s commercial story has always been a race between clinical promise and logistical friction. The original IV infusion required patients to visit an infusion center every two weeks, a barrier that slowed early uptake and sent Biogen’s stock down sharply when the February 2024 launch numbers disappointed investors.
The FDA’s July 13, 2026 approval of LEQEMBI IQLIK, a subcutaneous (under-the-skin) weekly self-injection that patients can administer at home, removes that barrier for the initiation phase of treatment. Management said on the earnings call that reimbursement expansion for the subcutaneous version is the next unlock for volume growth. The 67% year-over-year sales increase in Q3 suggests the market is already responding, though the absolute dollar figure still represents a small fraction of the drug’s theoretical addressable population.
At the Morgan Stanley healthcare conference in September 2026, CEO Chris Viehbacher noted the company has ten Phase 3 clinical trials beginning to read out, with potential for multiple product launches from several molecules, according to the conference transcript. The nearest catalyst: Phase 3 data for litifilimab in lupus, due in early December 2026.
What the analysts are arguing about
After the Q3 report, the analyst community split in a way that maps neatly onto the math-versus-mood tension. Robert W. Baird and Needham both raised their valuation targets to $290 on October 29, 2026, citing the raised guidance and growth portfolio momentum, according to Benzinga. JPMorgan upgraded the stock to overweight, lifting its target to $300, citing pipeline catalysts. BofA raised its target only modestly while maintaining a Neutral rating. Morgan Stanley, citing the same Q3 results, actually trimmed its target to $231, per GuruFocus.
The disagreement is not about the quarter. Everyone agrees Q3 beat estimates. The disagreement is about what the pipeline is worth before the trials report. At the current price of $253.62, our data show the stock trading at a price-to-earnings multiple of 28.9, against a decade median of 16.1. A multiple is simply the price a buyer pays for each dollar of annual profit. Biogen’s current price tag is nearly double what the market has historically paid for each dollar of its earnings.
If earnings land near the midpoint of the filed guidance range and the multiple compresses toward its decade median, the arithmetic points toward a price well below today’s. If the pipeline delivers and earnings grow into the current multiple, the opposite is true. The data cannot tell us which scenario unfolds. What the data can say is that the current price assumes the optimistic path, and optimistic paths in drug development have a well-documented habit of taking longer than expected.
Reading the numbers
- Revenue trend, FY2020 to FY2025: Filed across annual 10-K reports, revenue fell from $13.4 billion in 2020 to $9.7 billion in 2024, then ticked up 2.2% to $9.9 billion in 2025. In everyday terms: imagine a store that rang up $134 in sales on Monday and $97 by Friday, then sold $99 worth on Saturday. The Saturday uptick is real, but the week as a whole was still a loss. The analytical question is whether Saturday marks the start of a new trend or a one-day bounce.
- Q3 2026 revenue of $2.8 billion, up 11% year-over-year: Filed in the 10-Q for September 30, 2026. This is the fastest quarterly growth rate in several years, driven by Apellis products and Leqembi. An 11% quarterly gain on a nearly $3 billion base means roughly $280 million in new revenue appeared in one year. That is meaningful, but it needs to persist across multiple quarters to change the multi-year revenue story.
- Net debt of $6.4 billion as of September 30, 2026: Filed in the 10-Q for the quarter ended September 30, 2026. Against $2.2 billion in annual operating cash flow (the 2025 filed figure), this is about three years of cash generation owed to creditors. A household earning $80,000 a year with $240,000 in debt is not in distress, but it is not free to spend freely either. Acquisitions and pipeline bets compete with debt repayment for every dollar the business earns.
- P/E of 28.9 versus decade median of 16.1 (98th percentile), our data: A price-to-earnings multiple is the price paid per dollar of annual profit. At 28.9, the market is paying nearly $29 for every $1 Biogen earns. Historically, it has paid about $16. That gap is not automatically wrong, but it requires the business to grow earnings substantially to justify it. IF earnings stay flat and IF the multiple drifts back toward its decade median, the arithmetic implies a price roughly 44% below today’s level, one of two scenarios the current valuation gap forces any analysis to hold simultaneously.
- Non-GAAP EPS guidance for full-year 2026 of $12.20 to $13.00: Raised from the prior lower bound of $12.00, per the Q3 2026 earnings call. Non-GAAP strips out items like acquisition charges and restructuring costs. The GAAP figure will be lower. At the midpoint of $12.60 and the current price of $253.62, the implied multiple is just over 20, still above the decade median but less extreme than the headline P/E figure.
